Prior to your first appointment, please sign a records release to authorize your previous doctor to send your medical records to us.

Please note, medical records cannot be emailed. We can accept these by fax (303) 674-0031, mail, or dropped off during business hours.

We accept most private insurance & Medicaid. We also offer self-pay options for uninsured patients.

Please call your insurance to confirm we are in network with your specific plan.
