Shane Whitehead, PA-C

Shane grew up in St. Louis, Missouri, and earned her Bachelor of Health Science degree from the University of Missouri-Columbia. She went on to complete her Master of Medical Science in Physician Assistant Studies at the University of Missouri-Kansas City School of Medicine. Shane began her PA career at Seattle Children’s Hospital in Seattle, Washington, where she worked in Pediatric Bone Marrow Transplant. She is committed to delivering high-quality, compassionate care to pediatric patients and their families.

In her free time, Shane loves adventuring with her husband and their energetic dogs, Addie & Daisy. Together, they enjoy trail running, mountain and road biking, gravel riding, skiing, hiking, stand-up paddleboarding, and exploring the outdoors in all seasons.

Jordyn Johnson, PA-C

Kenneth Kutalek, MD

Jordyn grew up in Amarillo, TX. After obtaining her Bachelor's degree in Biology at Southern Nazarene University, she went on to receive her Master’s of Physician Assistant Studies at University of Texas Medical Branch (UTMB) in Galveston, TX.

Jordyn began her PA career at UTMB in Women's Health then transitioned into Pediatrics. She loves caring for children & supporting families.


In her free time, Jordyn enjoys hiking, knitting, watching movies, & traveling with her fiancé and westie puppy, Maple.

Dr. Kutalek graduated with a degree in Biophysics from Johns Hopkins University and obtained his Medical Degree from Mt. Sinai School of Medicine. He had post-graduate training in Pediatrics at both Children’s Medical Center in Dallas and at Children’s Hospital Colorado in Denver, and is board certified in Pediatrics. He has been in private practice in Evergreen since 1985

He is an Associate Clinical Professor at University of Colorado School of Medicine and has taught Medical students, Pediatric Residents, Physician Assistant students and Nurse Practitioner students. He is a strong children’s advocate, has run clinics for the disadvantaged, and was named Physician of the Year by the Coalition for the Medically Underserved.

Dr. Kutalek believes strongly that the ability to obtain high quality medical care is the unconditional right of every individual, and has fought for three decades to make that a reality. He is an open-minded life-long learner as well as an expert in finding bunnies, elephants and monkeys in little kids’ ears.

Leisure time (what’s that?) is spent reading sci-fi, rebuilding computers, gazing at the heavens with his 16” telescope, but mostly, traveling with the love of his life, Lynn. He has two grown children, David, a computer engineer, and Jenni, resident artist and designer.
